After completing a questionnaire, your answers may generate an educational symptom-based report with predictive pattern scores, visual bar graphs, suggested areas of focus, and lab categories that may deserve review before testing or consultation.

The WalshDoc Difference

Choose the Right Questionnaire

Select the questionnaire that best fits the patient and the main clinical question. The standard biotype questionnaire is most useful when the patient can describe long-standing mood, anxiety, focus, sleep, behavior, and stress-tolerance symptoms. Younger children usually do better with the pediatric neurodevelopment questionnaire.

Adult, teen, or older child Start with the Biotype Questionnaire when the patient can describe symptoms and history.
Toddler or young child Use the Pediatric Neurodevelopment Questionnaire for speech, sensory, sleep, gut, immune, behavior, regression, or autism-related concerns.
Known exposure or detox concern Use Toxic Burden or Biotype + Toxic Burden when chemical sensitivity, mold, mitochondrial stress, or clearance problems are central.
Already completed a report Use a follow-up questionnaire to track progress, side effects, setbacks, labs, and remaining symptoms.
