What Does the Cognitive Gut & Celiac Panel Test?
This panel combines a serum zonulin test with tissue transglutaminase antibodies, tTG IgA and tTG IgG. Together they investigate intestinal-barrier biology while screening for antibody patterns associated with celiac disease.
These are two related but different questions. Zonulin is used as an adjunct marker when intestinal permeability is suspected, while tissue transglutaminase antibodies are established serologic tests used in the evaluation of celiac disease.

Zonulin is involved in regulation of the tight junctions between intestinal cells. These junctions help control what passes from the intestinal tract into the circulation.
Higher serum zonulin measurements have been associated in some studies with disorders involving intestinal-barrier dysfunction and inflammatory or metabolic disease.
However, serum zonulin should be interpreted as an adjunct marker rather than a stand-alone diagnosis of “leaky gut.” Commercial assays have important limitations, so the result is most useful when interpreted with symptoms, history and other gastrointestinal findings.
tTG IgA & tTG IgG
Tissue transglutaminase antibodies are used to screen for celiac disease, an autoimmune disorder in which exposure to gluten triggers an immune response that can damage the small intestine.
tTG IgA is the preferred initial serologic test for most patients being evaluated for celiac disease.
tTG IgG can provide additional information when IgA deficiency is present. When celiac disease is strongly suspected, total IgA and additional testing may also be appropriate.

tTG Antibodies
Evaluate an immune response associated with celiac disease. A positive result may require additional clinical evaluation and, depending on the circumstances, confirmatory testing.
Important: Celiac Testing Should Be Done While Eating Gluten
Celiac antibody testing is most informative while the patient is still consuming a gluten-containing diet. Starting a strict gluten-free diet before testing can cause antibody levels to decline and may make celiac disease more difficult to detect.

Serum zonulin is sometimes used as an adjunct marker related to intestinal-barrier function. It should not be considered a definitive diagnostic test for leaky gut because currently available serum assays have important limitations.
What is the best blood test for celiac disease?
tTG IgA is the preferred initial serologic test for most patients. Total IgA is often considered as well because IgA deficiency can make IgA-based testing less reliable.
Why test tTG IgG?
tTG IgG is most useful when a patient has IgA deficiency. It is generally less useful as a stand-alone celiac screening test when total IgA is normal.
Does a positive tTG test prove celiac disease?
A positive antibody test raises suspicion for celiac disease but should be interpreted in clinical context. Depending upon the result, age and clinical circumstances, additional serology or intestinal biopsy may be needed to establish the diagnosis.
Should I stop eating gluten before a celiac blood test?
