About Second Opinion Physician

Walsh Approach Telemedicine, Biotype Assessment and Functional Medicine Strategy

Second Opinion Physician is a telemedicine practice founded by David Epstein, D.O., focused on the Walsh Approach, lab-guided nutrient therapy, methylation analysis, toxic burden assessment, and functional medicine strategies for mood, behavior, neurodevelopmental, and cognitive concerns.

The practice helps patients organize symptoms, history, prior treatment response, and laboratory findings into a practical clinical strategy. The goal is not a one-size-fits-all supplement list, but a clearer understanding of which biochemical and functional barriers may be driving symptoms or limiting response.

Founder background

David Epstein, D.O.

Dr. David Epstein is an osteopathic physician with more than 35 years of clinical experience and a long-standing interest in natural, nutritional, oxidative, and functional approaches to chronic health problems.

Before developing Second Opinion Physician as a telemedicine practice, Dr. Epstein founded and operated a medical clinic in Atlanta that emphasized EDTA chelation, IV vitamins, oxidative therapies, heavy-metal burden, antioxidant support, and functional medicine.

His interest in biochemical psychiatry deepened after training directly with Dr. William J. Walsh and learning the Walsh Approach to mood and behavior disorders.

Clinical direction

From symptoms to strategy

The Walsh model gave Dr. Epstein a more structured way to connect symptoms, traits, nutrient imbalances, methylation patterns, copper-zinc regulation, oxidative stress, and lab-confirmed treatment priorities.

Second Opinion Physician now uses that foundation alongside functional medicine review of diet, gut absorption, inflammatory burden, toxic exposure, hormones, compliance and follow-up trends.

What the practice focuses on

Walsh Approach + Functional Medicine Review

The Walsh Approach, pioneered by William J. Walsh, Ph.D. and the Walsh Research Institute, organizes many mood and behavior disorders around recurring biochemical patterns. Second Opinion Physician uses this framework as a starting point, then expands the review to include the functional barriers that may affect outcome.

5Biotype assessment

Symptoms, traits, history, labs and treatment response are reviewed together.

LABLab-guided strategy

Copper, zinc, ceruloplasmin, histamine, pyrroles, homocysteine, methylation and oxidative stress markers may be reviewed.

+1Functional barriers

Diet, gut absorption, toxic burden, inflammation, oxidative stress, hormones and compliance may affect outcomes.

Second Opinion Physician framework

The “5 + 1” Perspective

The Walsh model provides five core biochemical pattern categories. Second Opinion Physician adds a practical “+1” review for toxic burden, oxidative stress, methylation bottlenecks, gut-liver-kidney clearance, cellular resilience and other barriers that can limit response.

Five Walsh Pattern Categories

Core biochemical patterns used to organize symptoms, traits, labs and nutrient priorities.

UM
UndermethylationMethylation / histamine pattern
OM
OvermethylationMethylation sensitivity pattern
CO
Copper OverloadCopper-zinc / oxidative stress pattern
PYR
PyroluriaZinc / B6 / stress tolerance pattern
TO
Toxic BurdenDetox / oxidative stress / methylation burden

The “+1” Functional Review

This added layer asks why a clear biotype may still need a different order of support.

Methylation bottlenecksSAH, homocysteine, methionine and methylation demand
Creatine demandSupport strategy when methylation reserve is limited
Mineral transportCopper, zinc, ceruloplasmin and pyrrole patterns
Cellular resilienceOxidative stress, mitochondria and antioxidant reserve
Clearance and complianceGut, liver, kidney, diet, tolerance, timing and priorities
The practical goal is to identify the likely pattern, confirm the labs, remove response barriers, and track what changes over time.
Walsh Research connection

Video Interview with Dr. Walsh

Dr. Epstein’s video discussion with Dr. William Walsh on methylation and related treatment strategy is an important credibility piece for this page. It connects the foundation of the Walsh Approach with Dr. Epstein’s current focus on SAH, creatine demand, alkalinity, kidney pH, and methylation bottlenecks.

WalshDoc system

Questionnaires, Reports and Follow-Up Tracking

WalshDoc is the structured questionnaire and assessment system developed by Second Opinion Physician to make complex patient history easier to organize. It helps convert symptoms, traits, labs, and follow-up changes into readable pattern-based reports.

1

Questionnaire

Patients complete a focused symptom and history intake.

2

Pattern review

WalshDoc organizes answers into biotype, toxic burden, pediatric, insomnia or functional health domains.

3

Testing priorities

The report helps identify which labs may clarify the pattern.

4

Physician report

Dr. Epstein can add clinical impressions, supplement strategy and follow-up priorities.

5

Follow-up

Symptoms, labs and supplements can be tracked over time.

Why patients use SOP

Assessment, Testing and Practical Priorities

Second Opinion Physician is designed for patients who need more than a generic supplement list. The goal is to organize symptoms, testing, treatment response and follow-up strategy into a clearer clinical path.

Walsh-focused testing

Online access to biotype, methylation, copper-zinc, pyrrole and oxidative stress testing pathways.

Detailed assessment

Symptoms, history, labs, prior treatment response and functional barriers are reviewed together.

Medication nonresponse

Helpful for patients who have not responded well to medication-only or one-size-fits-all approaches.

Functional review

Diet, digestion, toxicity, inflammation, hormones, oxidative stress and nutrient tolerance are considered.

Supplement priorities

Key nutrients are separated from secondary supports so patients understand what matters most.

Follow-up strategy

Retesting, symptom tracking, maintenance nutrients and response trends can guide next steps.

Common concerns reviewed

Mood, Behavior, Neurodevelopment and Cellular Stress

The practice commonly reviews mood, behavior, cognitive, neurodevelopmental, metabolic, inflammatory and functional health concerns where nutrient biochemistry, methylation or oxidative stress may be relevant.
